Customer Order Form
  1. Select the items you wish to order from the Products section. Your complete order will appear below where, if you wish, you may change the selected order quantities.
  2. ENTER or Edit your Customer Shipping Information.
  3. You may go back to the Products page at any time to add further items to your order.
  4. Press the "COMPLETE TRANSACTION" button to send your order to us. NOTE: this button will only appear after all required information has been entered in your Customer Shipping Information.
  5. We will notify you of receipt of your order by return email.
  6. Shipping will be added to your mailed invoice. If you need to know the shipping cost of your order, please call us at 1-800-332-6566
